When a retiree or veteran passes, the family often has difficulties finding necessary military documents. Some of these documents are required validate eligibility for burial in a State or national veterans cemetery. The document search sometimes delays planning of funeral services.
In talking to Col (Ret) Ronald Han, Director of the State Office of Veterans Services, he provided information about the VA Form 40-10007, Application for Pre-Need Determination of Eligibility for Burial in a VA National Cemetery.

Complete the application form and attach copies of your qualifying documents – DD Form 214, NGB Form 22, 20 year certificate – and submit. The form and documents can be submitted by email, mail or fax; the specifics are on the form.
After the Department of Veterans Affairs completes their review, you will receive an eligibility certification for internment on VA letterhead. Families pass this letter of authorization to the mortuary staff as they plan the retiree or veteran’s services.
